New Patients

Patients coming in for their first visit should download our New Patient Packet, fill it out, and bring it to your first appointment. This paperwork will help ensure that your first office visit flows smoothly.  Also, if you are on any medication, please be ready to provide us the names of the medication and specific doses.

Existing Patients

To connect to your patient information, there is a secure portal for online access.  Please contact our staff for additional information.

Click the Patient Portal link in the upper right corner to:

  • Access test results
  • View parts of your medical chart
  • Verify appointments
  • Send a message to our practice

Policies Regarding Appointments

If you need to change or cancel your appointment, please give 24 hour notice by calling our office at: 303-425-9245 and pressing 2 to be connected to the scheduler.

At every appointment, please bring your insurance card(s) and co-pay if applicable. Co-pays are to be paid the day of visit and prior to seeing the practitioner or receiving service.

Insurance Information

We currently accept many private insurance plans and Medicare.

If you have questions about your insurance and wish to speak to our Benefits Specialists call our office at: 303-425-9245, press 8 and ask to speak with Deb.
