For Patients

New Patients

Patients coming in for their first visit have two options to complete their New Patient Paperwork.

If you have provided our office with an email address, you will receive an email inviting you to fill out your New Patient Paperwork electronically. If you complete your paperwork prior to your appointment, this will make your check-in process faster and smoother.

If you are unable to fill out your New Patient Paperwork prior to your appointment, please arrive at least 20 minutes prior to your appointment time so that you can fill out your paperwork in office.

All New Patients should bring their: insurance card(s), medication list, and any pertinent records or notes you would like your physician to look over.

Existing Patients

If you have provided our office with your email address, you will receive an email inviting you to our Breeze patient portal. Through this online portal you can review your medical records, message with your care team, and update your address, phone number, billing information and more!

If you are unable to access our Breeze patient portal, please let the front desk know when checking in if any of your information needs to be changed. This includes: your address, phone number, billing information, and insurance(s).

Travel Clinic Patients

If you are coming to visit our Travel Clinic you have two options to fill out your Travel Patient Paperwork.

If you have provided our office with an email address, you will receive an email inviting you to fill out your Travel Paperwork electronically. If you complete your paperwork prior to your appointment, this will make your check-in process faster and smoother.

If you are unable to fill out your Travel Paperwork prior to your appointment, please arrive at least 20 minutes prior to your appointment time so that you can fill out your paperwork in office.

Please take into consideration that all members of your Travel party will need to fill out this paperwork, so please plan accordingly. Please bring any pertinent vaccine records or other documentation that you feel would be important for your physician to look over.

Telehealth Patients

If you are scheduled for a Telehealth appointment with WIDC, you have the following options.

If you have provided our office with an email address, you will receive an email inviting you to fill out your Telehealth paperwork electronically.

If you are unable to fill out your paperwork electronically, you will be provided with a printed version to fill out that can be faxed to our office.

Infusion Center Patients

If you are scheduled for an appointment in one of our Infusion Centers, please arrive to your appointment on schedule. We have a limited number of chairs and we want to ensure that we can provide you with the highest level of care.

If you have already filled out patient paperwork after July 1st, 2019, please inform staff of any changes that need to be made to your account including: address, phone number, billing information, and insurance(s).

If you have filled out paperwork prior to July 1st, or if you are new to WIDC, you will have two options to complete your Patient Paperwork.

If your first visit to WIDC is taking place on a weekend, please arrive at your scheduled time and staff will assist you with your check-in process.
